Insurance Guarantee Scheme and the SPIKPA requirement for domestic helpers

Employers of foreign workers face two separate insurance requirements: an Immigration security guarantee, and a hospitalisation scheme. Domestic helper employers need to check both carefully.

Employers of a foreign domestic helper in Malaysia end up dealing with two separate insurance-style requirements that are easy to confuse: an Immigration Department guarantee scheme, and a hospitalisation and surgical scheme. They serve different purposes, are bought from different insurers or the same insurer under different names, and, importantly, one of them explicitly does not apply to domestic helpers at all.

The Foreign Workmen Insurance Guarantee (FWIG)

The Foreign Workers Insurance Guarantee, sometimes marketed as the Foreign Worker Insurance Scheme or Insurance Guarantee, is a security deposit required by the Immigration Department under Regulation 21 of the Immigration Regulations from employers hiring foreign workers. It functions as a guarantee to the Immigration Department that repatriation expenses will be covered if a foreign worker needs to be sent back to their country of origin during their stay in Malaysia. Several general insurers, including Progressive, MSIG and Berjaya Sompo, sell this cover to employers as a way of meeting the Immigration Department's security deposit requirement without tying up cash directly.

The point that matters for a domestic helper employer specifically: at least one insurer's own description of this scheme states plainly that it applies to foreign workers "excluding Domestic Maid". This means the FWIG, as an Immigration security guarantee, is generally not the product that covers a domestic helper; it is designed around workers in sectors like manufacturing, construction and plantation. If you employ a domestic helper, you should confirm directly with your insurer or agent whether a domestic helper requires this specific guarantee product, or whether a different Immigration security requirement applies to domestic helper employment instead, since the requirements for domestic helpers are administered somewhat separately from general foreign worker employment.

SPIKPA: the hospitalisation and surgical scheme

Separately from the Immigration guarantee, foreign workers, including domestic helpers, are generally expected to be covered under a hospitalisation and surgical scheme referred to in the market as SPIKPA (also seen as SKHPPA depending on the insurer). This is a standardised scheme sold by several general insurers and takaful operators, designed to cover hospitalisation and surgical charges if the foreign worker is admitted to a non-corporatised Malaysian government hospital due to accident or illness. As one insurer's published terms illustrate, this typically includes daily hospital room and board up to a set amount per day for up to 30 days, intensive care for up to 15 days, and an overall annual claim limit, commonly RM20,000, with cover explicitly described as designed for employers across manufacturing, construction, plantation, agriculture, services and domestic help. This is the scheme most directly relevant to protecting a domestic helper's access to medical treatment while employed in Malaysia, and it sits alongside, not instead of, any statutory social security protection.

Where PERKESO fits in

Separate again from both of the above, domestic workers, whether Malaysian or foreign, have been covered under the Employees' Social Security Act since 1 June 2021, giving them access to PERKESO's Employment Injury Scheme and other protections under the same framework as formal employees. Registration and contribution to PERKESO are mandatory for employers of domestic workers, with a defined contribution rate split between employer and worker, and failure to register or pay carries a penalty of a fine of up to RM10,000, imprisonment of up to two years, or both. This statutory scheme covers workplace accidents and occupational disease; it does not replace the hospitalisation and surgical cover described above, which is aimed at illness and accidents more broadly, not only those arising from work.

Putting the three together

For an employer of a foreign domestic helper, the practical picture is roughly:

  1. Check with Immigration or your agent whether a security guarantee is required for a domestic helper specifically, and if so, through which channel, since the standard FWIG product explicitly excludes domestic maids according to at least one insurer's terms.
  2. Arrange a hospitalisation and surgical scheme (SPIKPA/SKHPPA) for the domestic helper, since this cover is generally described as extending to domestic help and is the layer that responds to illness or accident requiring hospital admission.
  3. Register the domestic helper with PERKESO as required since June 2021, and keep contributions current, since this is a separate statutory obligation with its own penalties for non-compliance.

Because terminology varies between insurers, marketing material referring to "insurance guarantee" alone, without specifying whether it is the Immigration security deposit or a hospitalisation scheme, is worth clarifying before you buy. Ask specifically which regulation or requirement the product is meeting, and get that confirmed in writing from your insurer or agent.
