MediShield Life: the national plan everyone already has
Every Singapore Citizen and Permanent Resident is already covered by MediShield Life. Here is what it actually pays for, what it excludes, and where its limits sit.
If you are a Singapore Citizen or Permanent Resident, you almost certainly already have MediShield Life, whether or not you have ever thought about it. It is the base layer of the country's health insurance system, and understanding what it does and does not cover is the first step before deciding whether you need anything on top of it.
What it is
MediShield Life is a national health insurance scheme, administered by the CPF Board, that helps pay for large hospital bills and a defined set of costly outpatient treatments. Everyone is automatically covered β there is no opt-out and no medical underwriting β and premiums are payable in full from MediSave, which means most people never write a cheque for it directly. That universal, no-underwriting design is precisely what makes it different from any private health plan you might add later: MediShield Life does not exclude pre-existing conditions.
What it actually pays for
Cover falls broadly into three groups:
- Hospitalisation and day surgery, with daily claim limits that vary by ward type and by whether you are in a normal ward, an ICU, a community hospital, or receiving palliative care, plus separate limits for surgical procedures graded by complexity.
- A defined list of outpatient treatments, including cancer drug treatment, radiotherapy, kidney dialysis, immunosuppressants for organ transplant, and a handful of other named high-cost therapies β each with its own monthly or per-treatment limit.
- Selected high-cost treatments, such as certain cell and gene therapy products and drug treatments for specific blood and childhood-onset conditions, again subject to named limits.
There is a maximum claim limit per policy year, but no lifetime cap on the total you can claim over your life. Within each claim, you also pay a deductible once each policy year and a co-insurance percentage on top of that, which decreases as the claimable amount rises β so MediShield Life shares the cost of a bill with you rather than paying it in full.
What it never covers
MediShield Life applies a standard list of exclusions to every policy, regardless of insurer, because it is a single national scheme. Items on that list include ambulance fees, cosmetic surgery, most dental work, vaccinations, most fertility treatment, most maternity charges, treatment for injuries from a criminal act, and overseas medical treatment, among others. These exclusions exist because the scheme is designed around insurable, catastrophic-cost events rather than routine or elective care β a distinction worth remembering when a GP visit, a vaccination, or a planned procedure abroad turns out not to be claimable.
Where the coverage stops short
MediShield Life is calibrated to subsidised treatment in public hospital wards up to Class B2/C. If you are treated in a higher ward class, in a private hospital, or as a Permanent Resident whose bills attract different subsidy treatment, your bill is pro-rated before MediShield Life computes its payout β meaning you will be covering a proportionally larger share yourself. This is the specific gap that an Integrated Shield Plan (IP) is designed to close for people who want a higher ward class, a private hospital, or choice of doctor; MediShield Life alone is enough only if you are comfortable staying within subsidised public wards.
Checking your own coverage
Because MediShield Life is automatic, most people have never seen their policy details. You can check your coverage, your MediSave balance, and how a claim would be computed by logging into the CPF Board's website with Singpass, and using the MediSave and MediShield Life claims calculator to estimate what a given bill would look like after your claim limit, deductible and co-insurance are applied.
Deciding whether you need more
The scheme covers you for life and cannot be cancelled by an insurer, which is a real strength β but it was never designed to fully fund private hospital care or every outpatient scenario. Before adding an IP or a rider on top, it helps to be clear about what you are actually buying: a higher ward class, choice of doctor, or cover for the co-insurance an IP itself still leaves you paying.
